I have it set to convert from .mkv to h.264 and yet it only outputs mp4. Any way to fix this?

Both file types are supported but the file name must be .avi .
Make sure you have latest xbox 360 dashboard updates:
xbox.com/en-US/support/systemuse/xbox360/console/systemupdates.htm
